Animal Hospital Anomalies

No equipment needed โ you can reject these on sight. All of them also speak with a low-pitched, distorted voice.
- Smiling with Creepy Eyes โ Large eyes with black pupils, heavy under-eye bags and a big drawn-on smile. Reject it โ tasering makes the smile vanish.
- Three Eyes โ Three mismatched glowing red eyes replace the patient's own; the real eyes are sometimes still visible behind them. Reject.
- Crooked Face โ A crooked, distorted smile with sharp teeth and mismatched high-set eyes โ the 3D eyeballs stay visible in every expression. Reject.
- X-rayed Face โ Small, wide circular eyes with black pupils, paired with realistic human teeth. Reject.
- Staring โ The head tracks your camera or another player, rotating up to about 60ยฐ each way. Guaranteed to turn Mimic if admitted โ reject.
- Hollow Face โ Dark hollow eyes, constant twitching and a hunched posture. Admitting it guarantees a Death Ritual โ reject.
- Twitching โ Twitching arms and head, an elongated neck and a hunched back. Guaranteed Mimic if admitted โ reject.

Take the photo at the window, then compare it against the patient standing in front of you.
- Incorrect Photo โ The developed photo differs from the real patient โ eye appearance, ear shape, ear colour, mouth shape or the whole face. Reject.
- Static Photo โ The photo comes out covered in static or grain. Reject with the shutters โ shooting or tasering only makes it faint, and admitting it starts a Death Ritual.
- Cursed Photos โ Picking a photo up before it finishes processing costs you 10 sanity โ always wait for it to develop. Six disturbing varieties are documented.

These look completely normal at the window and in photos โ the camera feed is the only thing that gives them away.
- Black Eyes โ The face is covered by a black bar with realistic eyes staring back out of it. Reject.
- Unnatural Body โ The body is stretched and distorted, reshaping every time you reopen the camera โ the community calls it 'boneless'. Reject.
- Staring at Cameras โ Looks directly into the CCTV at all times, even while walking. Reject.
- Void / Black โ The entire body renders as solid black, but only on the CCTV feed. Reject.
- Camera Twitching โ Twitches only in the camera footage, never at the window. Reject.
- Skinwalker / Mimic โ Shows up as a Mimic on camera only, after it reaches the check-in spot โ use the lobby camera for a side view. Reject.
- Mismatching Ears โ Has different ears or horns on camera than it does at the window. Reject.
- Mismatching Face โ A different face on camera than at the window โ place a Deployable Camera in the office to catch it. Reject.
๐น Enemies inside the building (9)
Technically a separate category from anomalies โ these get in and hunt you rather than queueing at the window.
- Bed Monster โ Hides under beds in Rooms 1-5 and 7 and drags in anyone entering its red zone (โ10 sanity, then โ5 every 2s; kills patients in 30s). Feed it Maple Syrup, an Organ or a Transplant.
- Head Banger โ Bangs its head on the reception side window on Shifts 2-5 and blocks the shutters. Hand it a coffee or let it break the glass โ sending it away without coffee costs 20 sanity.
- Mimic (formerly Skinwalker) โ An admitted patient that says "I'm hungryโฆ" then turns hostile (โ15 per hit, โ6 every ~0.5s). Taser, X-taser, shoot or fight it manually.
- Stalker โ A tall black creature at hallway corners โ the lights flicker, then you lose 10 sanity if you look. Look away; since 10 July it can also check in, and a photo dispels it.
- Hiders โ Paper-thin wall-camouflaged creatures spawning in groups of 1-6 that steal your held item or deal โ10 sanity. Use the extinguisher, taser or gun, or simply outrun them.
- Mass of Eyes โ A red fleshy ceiling mass โ don't look up, or lose 2-5 sanity every half-second of eye contact. Neutralise it with Eyedrops.
- Ghost โ Spawns only after a patient has died. Invisible and drains 2 sanity per second (4 as Psychologist) โ reveal it with the fire extinguisher, then taser, shoot or scan it.
- Camera Figure โ A 50% chance on any broken camera; it zooms in until it jumpscares you for โ20 sanity. Switch or close the camera, and repair cameras from underneath.
- Tendril โ A purple tentacled mass on the Room 8 surgery patient with a 45s timer. Surgery order: Scissors, Transplant, Scissors, Transplant, then 3 random treatments. Never shoot, taser or coffee it โ that kills the patient.
๐ Universal tells that work on any anomaly
- Anomalies cannot faint โ by any means, including fire โ anyone who walks into a burning room without collapsing is an anomaly.
- Tasering pays โ tasering an anomaly grants +2 sanity; shooting one grants none.
- The shift won't end โ if every patient is treated but the shift summary never appears, an anomaly is still inside the building.

How many anomalies are in Animal Hospital?
There are 18 documented anomalies in Animal Hospital as of July 2026 โ 7 you can spot at the reception window, 3 in the photos and 8 only on the CCTV cameras. Nine further hostile creatures (Bed Monster, Mimic, Stalker and more) are tracked separately as enemies and are listed further down this page.
How do you identify anomalies in Animal Hospital?
There are three detection channels. At the reception window you look for wrong faces, staring, twitching and a low distorted voice. In photos you compare the developed picture against the real patient. On CCTV you watch for bodies, faces or ears that only look wrong on camera. Anything that fails a check gets rejected with the shutters.
How many shifts are in Animal Hospital?
There is no fixed total โ shifts are an endless survival counter, and the first four act as tutorials. You lose when your sanity hits 0 or three animals die. Notable beats: Shift 2 unlocks the Emergency Wing and shop, Shift 3 unlocks the taser, Shift 4 triggers the Ambulance Event, Shift 6 is Dr. Harlow's last active shift, and Liz arrives around Shifts 11-12 with her UV camera.
Does Animal Hospital have an ending?
No. Animal Hospital is endless survival with no win state and no endings system โ you play until your sanity runs out or three animals die. Sites claiming a "Shift 7 ending" are wrong: after Shift 7 Dr. Harlow simply appears in the lobby and answers questions if you bring him Mint Tea.
Does Animal Hospital have codes?
No. Animal Hospital has no code redemption system at all โ there is no redeem box in the game and the developers have never issued a code. Any site listing working "Animal Hospital codes" is fabricating them. Cash and Animal Coins are earned by completing shifts instead.
What is Barney's secret in Animal Hospital?
Barney is a rabbit who visits across Shifts 2-7 asking for coffee, then to store a red briefcase, then a photo, then a scalpel, and finally to be hidden from the police. Shelter him in the Reception Office instead of turning him in and he rewards you with a second coffee machine (300s cooldown), then stops visiting. Turning him in to Officer Duckman gets the officer knocked out and earns you nothing.
What is in Ratthew's vault in Animal Hospital?
After Ratthew's final visit he leaves you a key and a note. Remove the cat poster in Room 5 to reveal a hidden vault โ the key opens it, and inside are a photo of Rattina, a "TOP SECRET" suitcase and a note reading "The experiment was a success, the 'Shadow' now sees everything."
